I think I've got this one solved. He's not dead and replaced, but he's using body doubles and lately the doubles have been on stage and on interviews a couple of times.
Here's the smoking gun, Axl and a double in the same picture:
See the guy sitting far right two rows behind Axl? Which one is real, which one is a double? Both use plastic surgery, a hat, huge sunglasses and mustache to cover facial features.
So the updated theory is this: Axl lost his voice mid-nineties due to singing too hard and too high. In order to get the millions dumped on Chinese Democracy album back, Geffen Records put up Sebastian Bach with the task of fixing GnR. The same label both John Lennon and Kurt Cobain signed and died to gunwounds 3 years after. Since Axl's voice hasn't recovered and he has nervous breakdown repeatedly, they started using doubles for the album and the live concerts. The new GnR is a Team set up by Geffen, consisting of the actual band, ghostwriters for the songs, doubles for Axl and Sebastian Bach keeping the mess together.

This also explains the behavior in the video links I posted earlier. In some of these cases a double has been caught to an interview or autograph collection, in some its Axl trying to mask his features to make the doubles work.

Bear in mind that Axl is not the same person he was 10 or 20 years ago either. The current Axl has gained a lot of weight (10-30 kilos) in the last 5 years, has gone through several plastic surgeries and can't sing the high tones anymore. It's also likely that he's on antidepressant medication (such as Prozac), causing the rapid weight increase and changes in behavior. But the Team is also using doubles, no doubt.
